About the Role
As the Case Manager for Parents as Teachers Program, you will Shine Hope as you provide case management services such as assessment, education, and support to eligible families.
You will provide ongoing support and make resources available to participants and their families such as information and referrals to additional support and services.
You will also deliver services based on the Parent as Teachers Program model, including home visits, parent support groups and periodic developmental screenings to eligible client families.
Provide families with information regarding child development and effective parenting techniques, provide families with information with sensitivity to cultural and socio-economic needs of each client, positively present this program and the need for such programs to the local community and serve as a resource for program collaboration, the community, and other BCFS programs.
What you will do As a member of our team you will have an impact on our clients through accomplishment of the following responsibilities: Recruit and engage eligible families to receive services and determine family eligibility.
Promote HOPES program services and provide educational services through instructional presentations to community groups and representing Buckner and the HOPES Program at community outreach fairs; perform other public functions as assigned.
Develop and manage a system to coordinate on-site and off-site programs and to aid with communication among program sponsors/collaborators.
Develop professional relationships with institutions and agencies that provide education, job training, living skills training, employment services, housing, and child care services in order to coordinate services for program participants.
Provide case management by delivering information pertaining to basic needs, education, and goal setting.
Participate in regular support with the client during frequent home visits, office visits, and telephone contact as required by state grant and Buckner policy and procedures.
Deliver approved developmental screening tools on all children in case load and document and report results.
Assist program participants with accessing community resources, including, but not limited to, assisting with program enrollments, assisting with obtaining housing, and other services.
Utilizing Parents as Teachers curriculum when conducting parent education in the home.
Manage caseloads and timely and accurately prepare various documentation for the purpose of providing information required for program compliance.
Establish appropriate relationships with families that are based on equality and respect.
Work with the families to build on the families' strengths to effect needed change.
Ensure the implementation and accessibility of appropriate resources to meet those needs through networking, community resources awareness workshops that offer services needed.

Requires proficient knowledge of and/or work experience using community resources.
Awareness of community resources, including social service and health providers required.
Requires ability to observe and make accurate assessments of situations and individuals and determine appropriate action required.
Requires ability to drive assigned vehicle(s) or personal vehicle, with appropriate state license, following all laws applicable; must provide proof of liability insurance and must be eligible to be insured under Buckner's insurance policy.
Must be age 21 or older to drive on behalf of Buckner Shining hope in others.
(Your friendly attitude and willingness to learn is more important than a year's experience!) Preferred Qualifications In addition to the amazing qualifications and experiences listed above, if we had our say, we would also look for: Prior work experience in education, child health and development, child abuse or neglect, and/or parent education preferred.
Knowledge and experience with in-home visitation preferred.
